Das vorgestellte experimentelle Polytrauma-Modell aus Schädelhirntrauma und Femurfraktur mit hämorrhagischem Schock bei der Maus erfüllt die Anforderungen an ein standardisiertes Tiermodell, welches neben definierter Lokalisation und Schwere der Verletzung, Quantifizierbarkeit und Reproduzierbarkeit adäquate Analogien und Rückschlüsse auf die klinische Situation eines Polytraumas beim Menschen zulässt und damit verbundene charakteristische klinische, pathophysiologische und histopathologische Veränderungen darstellt.
Die polytraumatisierten Tiere unterschieden sich dabei von denjenigen mit den Einzelverletzungen (Schädelhirntrauma bzw. Femurfraktur mit hämorrhagischem Schock) innerhalb der histopathologischen Untersuchungen durch eine signifikant erhöhte astrozytäre Gliose. Immunologisch zeigten die Tiere der Polytrauma-Gruppe einen signifikant erhöhten Anteil der CD4+CD8+-T-Lymphozyten und eine tendenzielle Erhöhung der natürlichen Killerzellen in der Milz gegenüber den anderen beiden Versuchsgruppen. Auch die TNF-α-Konzentration im Plasma wies eine tendenzielle Erhöhung verglichen mit den isolierten Verletzungen auf.
Die teilweise signifikanten Unterschiede der polytraumatisierten Tiere sind nicht lediglich damit zu erklären, dass das bei ihnen induzierte Trauma im Vergleich zu den Einzelverletzungen ein größeres Verletzungsausmaß darstellte, sondern es spiegelt die komplexen Zusammenhänge der beiden induzierten Traumata wider. Bei anderen gemessenen Parametern, wie beispielsweise der IFN-γ-Konzentration im
Plasma, scheint dagegen der hämorrhagische Schock deutlicher an der Erhöhung beteiligt zu sein, da in der Schädelhirntrauma-Gruppe zum gegebenen Messzeitpunkt lediglich bei einem Tier überhaupt IFN-γ im Plasma gemessen werden konnte, während bei den beiden Gruppen, bei denen ein hämorrhagischer Schock induziert worden war, mit Ausnahme von jeweils einem Tier pro Gruppe bei allen Mäusen IFN-γ nachweisbar war. Ähnlich verhält es sich bei der Abgrenzung der weißen von der roten Milzpulpa, die lediglich bei einigen Tieren der Schädelhirntrauma-Gruppe möglich war. Während bei der Untersuchung der Leber auffiel, dass die Tiere der Fraktur-Schock-Gruppe eine signifikant stärkere hydropische Degeneration zeigten. In diesem Fall müsste das zusätzlich zu dem hämorrhagischen Schock ausgelöste Schädelhirntrauma eine stärkere hydropische Leberdegeneration verhindert haben.
Bei allen Parametern, die nur bei der Organentnahme gemessen wurden, ist allerdings zu beachten, dass es sich um Momentaufnahmen handelt, die lediglich den Zustand der Tiere 96 Stunden nach Traumainduktion hinsichtlich der Aktivierung des Immunsystems durch gemessene Zytokinkonzentrationen und Lymphozytensubpopulationen darstellen. Es wäre daher vorstellbar, in weiterführenden Untersuchungen auch andere Messzeitpunkte zu berücksichtigen.
Des weiteren ist es denkbar, mit dem vorgestellten Polytrauma-Modell genetisch veränderte Mäuse zu untersuchen, um beispielsweise gezielte Fragestellungen hinsichtlich der Funktion und Auswirkungen bestimmter Zytokine im Rahmen des Polytraumas auch im Hinblick auf die Neurohistopathologie zu beantworten oder neue Therapieansätze zu prüfen.

Das Polytrauma stellt in Industriestaaten die häufigste Todesursache junger Erwachsener dar und ist bei Überlebenden oft mit Behinderungen und Problemen bei der sozialen wie beruflichen Reintegration verbunden. Damit hat das Polytrauma neben der medizinischen auch eine große sozioökonomische Bedeutung. Im Rahmen eines Polytraumas ist das Schädelhirntrauma eine der häufigsten Verletzungen. Es hat bedeutenden Einfluss auf den Krankheitsverlauf und führt zu einer deutlichen Verlängerung der Intensivzeit und der gesamten Rehabilitation sowie einer erhöhten Letalität. Die wechselseitige Beeinflussung von Schädelhirnverletzungen und extrakraniellen Traumata führt zu einem komplexen Krankheitsbild, dessen Mechanismen noch nicht ausreichend erfasst sind.
Um diese zu untersuchen sollte im Rahmen der vorliegenden Arbeit ein experimentelles Polytrauma-Modell entwickelt werden, das die klinische Situation schädelhirnverletzter Polytraumapatienten simuliert, die damit verbundenen pathophysiologischen und immunpathologischen Veränderungen darstellt und somit die Grundlage weiterführender Untersuchungen bildet. Mit diesem Ziel wurde an Mäusen des Inzuchtstammes C57BL/6 ein Schädelhirntrauma mit Hilfe eines
„Controlled Cortical Impact“ induziert und mit einer stumpfen Weichteil- und Knochenverletzung in Form einer geschlossenen Femurfraktur und einem volumenkontrollierten hämorrhagischen Schock kombiniert. Es sollte der Effekt des Schädelhirntraumas auf die pathophysiologischen Veränderungen nach traumatisch-hämorrhagischem Schock sowie der Einfluss dieser extrakraniellen Verletzung auf die Histopathologie des Gehirnes nach Schädelhirntrauma untersucht werden.
Mit Hilfe klinischer, histologischer und immunologischer Verfahren wurden bei einer Versuchsdauer von 96 Stunden die Auswirkungen des Polytraumas untersucht und
mit den Veränderungen der Einzelverletzungen (isoliertes Schädelhirntrauma bzw.
Femurfraktur mit hämorrhagischem Schock) verglichen.
Histologisch konnte eine Verstärkung der astrozytären Gliose nach Schädelhirntrauma durch einen zusätzlichen traumatisch-hämorrhagischen Schock nachgewiesen werden. Im Rahmen der immunologischen Untersuchungen wurde ein signifikant höherer Anteil der CD4+CD8+-T-Lymphozyten in der Milz bei der Polytrauma-Gruppe verglichen mit den anderen Versuchsgruppen und ein tendenziell höherer Anteil der natürlichen Killerzellen festgestellt. Sie zeigten ferner eine tendenziell höhere systemische TNF-α-Konzentration.
Das beschriebene experimentelle Polytrauma-Modell aus Schädelhirntrauma und Femurfraktur mit hämorrhagischem Schock bei der Maus lässt adäquate Analogien und Rückschlüsse zu der klinischen Situation eines Polytraumas beim Menschen zu und stellt damit verbundene charakteristische klinische, pathophysiologische und histopathologische Veränderungen dar. Es kann für weiterführende Untersuchungen zur Klärung spezifischer immunologischer und neuropathologischer Fragestellungen im Rahmen eines Polytraumas und möglicher pharmakotherapeutischer Ansätze genutzt werden.

Establishment of a murine multiple trauma model consisting of traumatic brain injury and femoral fracture with concomitant hemorrhagic shock
Multiple trauma is the most common cause of death of young adults in developed countries. Survivors often suffer from disabilities as well as social and vocational rehabilitation problems. Therefore, multiple trauma has medical relevance and also significant socio-economic importance. In the context of multiple trauma, traumatic brain injury (TBI) is one of the most frequent injuries. It has an extensive impact on the course of disease and leads to a considerable prolongation of the intensive care time and the whole rehabilitation process in addition to an increased lethality. The mutual influence of TBI and extracranial trauma leads to a complex clinical picture with mechanisms that are still inadequately investigated.
The present study aimed to develop an experimental multiple trauma model, which simulates the clinical situation of brain-injured multiple trauma patients and describes the associated pathophysiolgical and immunopathological alterations, in order to provide a basis for further research in this field. With this objective, TBI was induced in C57BL/6 mice by means of controlled cortical impact (CCI) and combined with blunt soft-tissue and bone injury that was performed by a closed femoral fracture with concomitant volume-controlled hemorrhagic shock.
The purpose was to measure the effect of TBI on pathophysiological changes following traumatic-hemorrhagic shock as well as the influence of this extracranial injury on the histopathology of the brain after TBI. During an experimental period of 96 hours, the effects of multiple trauma were tested by means of clinical, histological and immunological procedures and compared to the alterations of single injury (isolated TBI and femoral fracture with hemorrhagic shock respectively).
Histological testing showed an enhancement of the astrocytic gliosis after TBI, caused by the additional traumatic-hemorrhagic shock. On the basis of the immunological testing, in the group of polytraumatized mice a significantly increased
rate of CD4 CD8 -T cells in the spleen was measured in comparison to the other groups, as well as a tendency to an increased rate of natural killer cells. Additionally, a tendency to an increased level of systemic TNF-α concentration was observed.
The described murine multiple trauma model consisting of TBI and femoral fracture with concomitant hemorrhagic shock enables adequate analogies to clinical multiple trauma situations of human beings and visualizes associated characteristic clinical, pathophysiolgical and histopathological changes. It can be used for further research of specific immunological and neuropathological questions in the context of multiple trauma and possible pharmacotherapeutic approaches.
