Nach intravenöser Gabe erreicht Gamithromycin in der Lunge therapeutisch wirksame Spiegel,welche deutlich über der MHK90 für R. equi liegen, und scheint gut verträglich zu sein. Da jedoch nur zu einem Zeitpunkt eine BAL durchgeführt wurde, konnten weder die Halbwertszeit noch tmax/Cmax in der Lunge bestimmt werden. Daher kann der Konzentrationsverlauf in der Lunge nicht bestimmt werden und nur im Vergleich mit der i.m.-Studie eine Aussage dazu gemacht werden.
Vergleicht man die Plasmakonzentrationen mit denen aus der Studie von BERGHAUS et al. (2011), so fällt auf, dass diese zu Beginn nach der i.v.-Gabe deutlich höher liegen und sich gegen Ende den Werten nach der i.m.-Gabe annähern. Es ist zu vermuten, dass die kürzere Halbwertszeit nach i.v.-Applikation durch die höhere Gesamtkonzentration ausgeglichen wird und daher nach 168 Stunden, also gegen Ende des üblichen Dosierungsintervalls, vergleichbare Konzentrationen im Plasma nach i.v.- und i.m.-Gabe vorliegen. Aufgrund dessen lässt sich vermuten, dass auch die Konzentrationen in der Lunge einen ähnlichen Verlauf haben wie nach der i.m.-Applikation. Demnach wäre eine Dosierung von 6 mg/kg KGW i.v. einmal pro Woche ausreichend.
Obwohl die beiden Studien sehr ähnlich sind, die Probanden waren in einem ähnlichen Alter, die Dosierung war die selbe und die Probenentnahme, -aufbereitung sowie – messmethode waren vergleichbar, müssen Unterschiede oder Abweichungen im Ablauf bedacht werden, die aus der Veröffentlichung nicht ersichtlich sind und zu den Abweichungen geführt haben können. Dies ist zwar aufgrund der doch recht
ausgeprägten Unterschiede in den Ergebnissen als alleinige Ursache nicht allzu wahrscheinlich, aber auch nicht auszuschließen.
Aus diesen Gründen sind weitere Studien mit wiederholten BALs notwendig, um eine sichere Aussage zum Konzentrationsverlauf in der Lunge und somit zur Eignung von Gamithromycin i.v. für die Therapie der Rhodokokkose machen zu können.
Empfehlenswert sind drei BALs relativ früh, genauer 12, 24 und 48 Stunden nach der Injektion des Gamithromycins, um den Zeitpunkt der maximalen Konzentration in der Lunge zu erfassen sowie drei gegen Ende des Dosierungsintervalls, 120, 168 und 240 Stunden nach der Applikation, um die Halbwertszeit in der Lunge bestimmen zu können sowie um zu sehen, ob die Konzentration in der Lunge über das Dosierungsintervall von 168 h ausreichend hoch bleibt.
6 Zusammenfassung
Tanja Randow Pharmakokinetik und Verteilung von Gamithromycin nach intravenöser Injektion beim Fohlen.
In der vorliegenden Arbeit wurde die Pharmakokinetik von Gamithromycin nach intravenöser Injektion in Fohlen untersucht. Es ist bekannt, dass Gamithromycin nach intramuskulärer Gabe wirksame Konzentrationen gegen Rhodoccocus equi (R. equi) über einen Zeitraum von 7 Tagen erreicht. Weil aber die Verabreichung von Zactran® i.m. schwere Nebenwirkungen verursacht, die nach der i.v. Gabe nicht auftreten, sollten hier die Konzentration des Gamithromycins im Plasma und in dem Lungenkompartiment nach i.v. Gabe erfasst werden.
Die Studie umfasste zehn gesunde Warmblutfohlen denen das Gamithromycin in einer Dosierung von 6 mg/kg KGW als Bolus intravenös verabreicht wurde. Um den Konzentrationsverlauf im Blut zu bestimmen, wurde den Fohlen in festgesetzten Zeitabständen über 7 Tage Blut abgenommen. Da R. equi vorwiegend intrazellulär in der Lunge vorkommt, wurde nach vierundzwanzig Stunden eine BAL durchgeführt und sowohl in den hierbei gewonnenen Zellen als auch der PELF die Konzentration des Gamithromycins gemessen.
Die Gamithromycin-Konzentration wurde mit einer validierten LC-MS/MS Methode bestimmt.
Es konnte gezeigt werden, dass sich Gamithromycin nach intravenöser Gabe in der Lunge anreichert, die Konzentration in der PELF (CPELF 1046,3 ± 590,3 ng/ml) war zum Zeitpunkt der BAL 16fach so hoch wie die im Plasma (C24 72,6 ± 30,6 ng/ml), in den BALC (CBALC 31220, ± 8469,9 ng/ml) war sie sogar 470fach höher.
Vergleicht man die Werte mit denen nach intramuskulärer Injektion, so ist die Gesamtkonzentration nach intravenöser Gabe (AUC0168h 7,2 ± 2,4 µg x h/ml) deutlich höher als nach intramuskulärer Gabe (AUC0168h 4 ± 0,4 µg x h/ml). Dafür ist die terminale Halbwertszeit nach intravenöser Injektion kürzer (29,4 ± 5,1 h) als nach
intramuskulärer (39,1 ± 3,1 h), so dass gegen Ende der Studie die Konzentrationen im Plasma ähnlich sind.
Die Konzentration in den BALC lag 24 Stunden nach der intravenösen Verabreichung des Gamithromycins mit 31, 2 ± 8,5 µg/ml über denen nach intramuskulärer Gabe zum selben Zeitpunkt sowie über der MHK90 für R. equi, die 1µg/ml beträgt. Somit liegen zu diesem Zeitpunkt wirksame Konzentrationen in den bronchoalveolären Zellen vor.
Da nur zu einem Zeitpunkt eine BAL durchgeführt wurde, lassen sich keine Aussagen zum Verlauf der Gamithromycin-Konzentration in der Lunge machen. Es lässt sich aber vermuten, dass dieser ähnlich ist wie nach intramuskulärer Gabe, da die Plasmakonzentrationen und die Konzentration in den BALC zum Zeitpunkt der BAL über denen nach der intramuskulären Applikation liegen.
7 Summary
Tanja Randow The pharmacokinetics and distribution of gamithromycin after intravenous injection in foals.
In the present study pharmacokinetic data of gamithromycin after intravenous injection in foals were evaluated. It is known that gamithromycin reaches effective concentrations against R. equi over 7 days after intramuscular injection.
Ten healthy warmblood foals were included in the study and gamithromycin was administered intravenously as a bolus with 6 mg/kg of bodyweight. Blood samples were taken in defined time-intervals over 168 hours to determine the concentration course in the blood. Because R. equi is an intracellular pathogenic agent, after 24 hours a BAL was performed to determine the concentration in the PELF and the BALC.
The concentration of gamithromycin was determined using a validated LC-MS/MS-method.
The results show that after intravenous injection, gamithromycin accumulates in the lung. The concentration in the PELF (CPELF 1046.3 ± 590.3 ng/ml) was 16 times higher than in plasma (C24 72.6 ± 30.6 ng/ml) and in the BALC (CBALC 31220 ± 8469,9 ng/ml) it was even 470 times higher.
Comparing the data after intramuscular injection, the concentration in plasma is higher after intravenous injection (AUC0168h 7.2 ± 2.4 µg x h/ml) than after intramuscular injection (AUC0168h 4 ± 0.4 µg x h/ml). On the other hand the terminal half-life is shorter after intravenous (29.4 ± 5.10 h) injection than after intramuscular (39.1 ± 3.1 h), so at the end of the kinetic the concentrations in plasma are similar.
The concentration in the BALC 24 hours after the intravenous injection of gamithromycin with 31.2 ± 8.5 µg/ml was higher than after intramuscular application at the same time (8.9 ± 1.6 µg/ml) and the MIC90 for R. equi with 1 µg/ml. So at this point of time there are effective concentrations in the bronchoalveolar cells.
Because BAL was only performed at once, we cannot make any statements of the concentration course in the lungs. But it can be assumed that it is similar to the one after intramuscular injection because the concentration in the plasma and BALC was higher after intravenous application than intramuscular injection.
