SED S011 – Information on urgent vitally necessary treatment – outside Member State of residence
The institution of the Member State of residence uses this SED to inform the competent institution that it granted an authorisation for an insured person to receive an urgent vitally necessary treatment outside the Member State of residence.
The authorization cannot be refused by the competent Member State, in accordance with the second sentence of article 20 (2) of Regulation (EC) 883/2004.
The authorization (portable document S2) shall be granted by the Institution of the Member State of residence on behalf of the competent institution.
The following data are important here:
Starting-Ending date of entitlement section shows Start and Ending date proposed for the urgent treatment.
Attachments section shows all type of attachments allowed:
o Medical report;
o Authorization;
o Other documents.
Treatment section provides data about the treated case (treatment description, the place of treatment, etc.).
It is possible to add attachments to SED S011.
