SED S057 – Reply to Notification of Application for Cash Benefits – Long term Care

This SED has to be used by the Institution in the Member State of Residence (or Stay) to reply to the Competent Institution whether the insured person is entitled or not to long- term care benefits in kind.

Specific mandatory fields are related to request issue date and information on entitlement for long- term care benefits in kind:

- Request issue date;

- Entitled or not;

- Period;

- Amount, currency and frequency of payment;

- Description of the benefits.
