SED S010 – Reply to request for Entitlement Document - scheduled treatment outside Member State of residence
The institution in the competent Member State uses this SED to reply to the request of authorisation (of scheduled treatment outside the Member State of residence) performed by an institution in the Member State of residence. The reply contains the key decision whether the authorisation for scheduled treatment was granted or not.
The institution in the competent Member State issues a portable document S2 to the person concerned as well in case of authorization.
The reply contains the following important elements:
Decision - the competent institution indicates the decision by selecting one of the following options:
o "Authorisation for scheduled treatment is granted ";
o " Authorisation for scheduled treatment is not granted "
Additional information on treatment, such as treatment description, date of the request and provider information;
The presence of the entitlement document as an attachment to the SED.
When you indicated that it is not granted, you give the reason for refusing the extension of entitlement document. When the reason is 'Other reason', you may provide more information as to the reason why.
