SED S014 – Request for information if treatment is provided for by Member State of stay - scheduled treatment

This SED has to be used when an insured person asks an authorization to receive an appropriate (scheduled) treatment outside the Member State of residence / competent Member State.

In the Request for Information section the competent institution indicates the nature of its request for information by ticking the appropriate box(es), i.e. to know :

 To inform us if the treatment is provided for by your legislation;

 To inform us about the estimated costs of the treatment.

It is possible to tick both boxes.

In Treatment Specification section, the competent institution should mention additional information on:

 Description,

 Provider information (name and address,

 the name of the provider

It is possible to add attachments to SED S014.
