SED S012 – Acknowledgement of receipt – Information on authorisation for urgent vitally necessary treatment

SED S012 is used by the competent Institution in order to acknowledge the receipt of the information regarding the authorisation for an urgent vitally necessary treatment that the Institution of the Member State of residence granted..

In the Acknowledgement of receipt section you need to fill in the following fields::

 In "Your document S011 / S2 issued on" - fill in the date on which the SED S011 and/or the portable document S2 was issued;

 In " Date of receipt" fill in the date of reception of the SED S011.

Attachments are not allowed to SED S012.
