SED S040 – Request for Periods - Insurance Risk Type: Sickness, Paternity and Maternity

SED S040 is used by the competent Institution in order to determine relevant periods of insurance, employment, self-employment or residence completed by the person concerned under the legislation of another Member State.

In the Request section you need to fill in the following fields:

Type of benefit concerned - with choice between:

 Sickness

 Maternity

 Paternity

Nature of benefit concerned - with choice between:

 In cash

 In kind

Note that: Only one option can be chosen – if periods have to be confirmed for different type of benefits the process has to be started again and new S040 has to be issued.

Period concerned with:

 fix period (start and end date mandatory) or

 open period (only start date mandatory).

It is recommended to indicate the end date of the concerned period, in order to ensure that the reply will determine all the periods within the required time interval.
