Zum vierten Problem. Wie bereits erwähnt, werden arbeitsstoff
bedingte Herzschäden bei uns in der Herzforschung kaum berück
sichtigt (ausser bei den weitgehend gelösten Problemen Schwefel
kohlenstoff und Nitratester). Abgesehen von diesen Einzelfällen ist die Herzforschung fast ausschliesslich am Arteriosklerose- oder Risikofaktoren-Modell orientiert, bestenfalls mit Anleihen beim Stress-Modell. Diese Forschung hat den schon erwähnten 'blinden Fleck':
- einerseits betont sie die herzschädigenden Wirkungen von Alkohol, Zigarettenrauch und Cholesterin
- andererseits vernachlässigt sie die mögliche herzkreis
laufschädigenden Wirkungen von etwa 7o.ooo Arbeitsstof
fen, Hunderten von Nahrungszusatzstoffen, Tausenden von Medikamenten, und Zehntausenden von Umweltschadstoffen.
Dieser Widerspruch wird aber merkwürdigerweise nicht gesehen. Die systematische und umfassende Berücksichtigung von Schadstoffen aus Arbeits- und Umwelt könnte daher für die Herzforschung einen wich
tigen neuen Aspekt einführen ('new look'), der als Ausgangshypothese den inzwischen stark sinkenden Grenznutzen der Herzforschung kräftig anheben könnte.
Nach einem neuen und recht gründlichen Ueberblick über epidemiologi
sche Studien zu 'cardiovascular diseases and the work environment' (KRISTENSEN 1989), sind bisher folgende Zusammenhänge zwischen Ar
beitsstoffen und Herzkreislauf-Krankheiten empirisch gesichert:

Einige Arbeitsstoffe mit gut belegter herzkreislauf-schädigender Wirkung fehlen hier offensichtlich, wie z.B. Pestizide, Ozon, Reiz
gase, diverse Treibmittel für Aerosole, etc., aber auch Medikamente, und aktives Rauchen. KRISTENSEN erwähnt selber, dass andere, eben
falls gründliche Uebersichtsarbeiten (z.B. KURPPA u.a. 1984, ROSEN- MAN 1979) zu teilweise differierenden Einschätzungen kommen, und
etwa Kohlenmonoxid, Blei, Kadmium und vor allem organischen Löse
mitteln eine (noch) grössere Herzkreislauf-Schädlichkeit zuschrei
ben. Ebenso fehlt bei KRISTENSEN die Berücksichtigung 'chemisch sensitiver' Personen (ASHFORD/MILLER 1989), ebenso die Berücksichti
gung von hohen und Mehrfach-Expositionen. Schliesslich ist zu beden
ken, dass gesicherte Zusammenhänge nicht notwendig starke Zusammen
hänge sind, und dass ungesicherte Zusammen- hänge nicht notwendig schwach sind. - Trotz dieser Einschränkungen kann KRISTENSENs Dar
stellung einen ersten Anhaltspunkt geben. Sie zeigt, dass in vielen Bereichen der Arbeitswelt mit arbeitsstoff-bedingten Herzkreislauf
schäden zu rechnen ist.
Der 'verborgene Anteil' bzw. das 'attributive Risiko' von Arbeits
stoffen an den Herzkreislaufkranken und -toten lässt sich ohne

eigene Studien natürlich kaum abschätzen. Aber allein die Grössen—
Ordnung der Erkrankungs- und Sterbeziffern verdeutlicht, dass hier - rein numerisch - ein grosses präventives Potential liegt. So sterben allein in der 'alten' Bundesrepublik jährlich etwa 34o.000 Menschen an Herzkreislauf-Krankheiten. Würden Arbeitsstoffe auch nur 5 % die
ser Todesfälle verursachen, liessen sich demnach prinzipiell 17.000 Todesfälle vermeiden. Herzforschung mit einem entsprechenden 'new look' auf Arbeitsstoffe wäre somit sicherlich sinnvoll. Ob dies dann auch für Prävention und Gesundheitsförderung gilt, müsste in einem weiteren Schritt geprüft werden.
Ein Beispiel. - OLSEN/KRISTENSEN (1991) haben versucht, für Dänemark den 'ätiologischen Anteil' nicht-chemischer Arbeitsplatz-Faktoren für die Herztodesfälle zu berechnen. Obwohl ihre Studie chemische Faktoren ausser Acht lässt (unter anderem wegen fehlender Exposi
tionsdaten), errechnen sie, dass 16 Z der HerzkreislaufSterblichkeit bei Männern (vor dem Rentenalter), und 22 Z bei erwerbstätigen
Frauen prinzipiell durch Arbeitsgestaltung vermeidbar wäre. Wird auch 'sitzende Tätigkeit' einbezogen, erhöhen sich diese ätiologi
schen Anteile sogar auf 51 Z für Männer und 55 Z für Frauen. Sind diese Zahlen nur halbwegs realistisch - und gilt Analoges auch für chemische Schadfaktoren -, eröffnet sich hier also ein weites Feld für ernsthaften (nicht nur symbolischen) Gesundheitsschutz.
Eine weitere Ueberlegung: Viele herzschädigende Stoffe schädigen auch auf andere Weise die Gesundheit - entweder spezifisch durch Krebs, Nervenschäden, Immunschwäche, Störung der Fortpflanzungs
fähigkeit, etc.; oder unspezifisch durch Beeinträchtigung der
unterschiedlichsten Körperfunktionen. Wie KRISTENSEN ausführt, wäre daher selbst bei ungenügender oder unspezifischer Evidenz Prävention bei herzschädigenden Arbeitsstoffen sinnvoll, denn
"(i) if one chooses not to act until one has loo Z certain evidence, one is likely to make mistakes which have serious consequences for the health and mortality of many people, and
(ii) the factors which have been mentioned ... are all risk factors for diseases other than CVD (=cardiovascular diseases;
W.M.). If one or more should prove not to be a risk factor for CVD, there would still be a positive effects from reducing or removing these factors". (1989, S. 257)
Diese Argumentation wird zwar sicher oft auf Ablehnung stossen, da ein entsprechendes Vorgehen als wissenschaftlich naiv, ökonomisch uneffektiv und politisch nicht durchsetzbar gilt. Doch zeigen Bei
spiele sehr wohl die Sinnhaftigkeit und mögliche gesellschaftliche
Akzeptanz einer solchen Strategie - etwa beim Rauchen, wo die krebs
erzeugende Wirkung immer wieder infrage gestellt wird, und dennoch ein gesellschaftlicher Konsens besteht, dass bereits die anderen gesundheitsschädlichen Wirkungen des Rauchens drastische Eingriffe rechtfertigen; oder beim Blei, das radikal aus dem Treibstoff ver
bannt wurde, obwohl seine umweltschädliche Wirkung noch lange nicht gesichert war.
