DA062 -Request for Authorisation of Transport to the Place of Residence or to a Hospital
Purpose of the SED:
It is necessary to receive authorization from the competent Member State for transport to the place of residence or to a hospital.

Data required:
Specific mandatory field in DA062 (without which SED cannot be send):
Section 5.1 “Destination of transport” – with two possibilities consistent with the regulation;
Section 5.2 “Hospital” – which becomes mandatory, if in the destination of a transport “to the following hospital” was chosen;
Section 6 “Medical report is attached” – with choice between yes or no.
If in the destination of the transport “place of residence” was chosen persons address must be provided in section 2.1.
