DA051 - Notification that Conditions for an Occupational Disease are not Fulfilled and Forwarding of Declaration or Notification

Purpose of the SED:

The Member State forwards to the previous Member State the notification or declaration about not fulfilling the conditions and all accompanying certificates, findings, reports of medical examinations.
