DA050 - Send Declaration or Notification of Occupational Disease

Purpose of the SED:

DA050 is a declaration or notification of the occupational disease to the Member State that is considered to be the competent one (the last one where such an activity likely to cause the disease, was pursued)

DA050 allows to give information about all possible periods of exposure.

This declaration should include information about the employer, the periods of employment, and the possible causal link between the activity and disease.

Data required:

Section 5 “Information on periods of exposure” is mandatory and repeatable and following data are required:

 Employer name;

 Employer identification number with a type;

 Description of the job;

Providing information on causal link and the employment period are not mandatory, as they might not always be known, but if available must be provided.

DA055 - Notification of Final Decision on Appeal Purpose of the SED:

 If the final decision did not confirmed the initial decision to refuse benefits in the Member State the institution of that Member State is obliged to reimbursed the advance

DA057 - Request for Information on Aggravation of Occupational Disease Purpose of the SED:

The institution in a Member State to which a claim was submitted contacts the respective institution in any other previous Member State in order to obtain the information it

DA058 - Information on Aggravation of Occupational Disease Purpose of the SED:

The Member State replies to the SED DA057 with SED DA058 providing details about the aggravation of the occupational disease in order to assess the right to the supplement.. For
