DA052 - Occupational Disease - Notification of Appeal Purpose of the SED:
The Member Stare against which decision to refuse benefits an appeal was lodged informs the Member State to which the declaration or notification was forwarded to about the appeal. The appeal should be attached.

Data required:
Information about lodged appeal must be provided (section 5), including:
Date of the decision against which an appeal was lodged;
Date of the appeal;
Information about the copy of the appeal.
