SED S045 - Entitlement Document – Temporary Stay

This SED allows the Competent Institution to inform whether the person concerned is or is not entitled to health benefits in kind from this Member State.

If a person is entitled to health benefits in kind during his/hers temporary stay in another Member State, the Competent Institution provides information about the entitlement by filling the period of entitlement in Section 3.1. on the S045. S045 is considered as an entitlement document equally to PRC and EHIC. It is not foreseen to attach a provisional replacement certificate (PRC), as the PRC itself is not a part of EESSI. The PRC is a paper document, typically issued if the insured person directly gets in touch with the competent institution.

If a person is not entitled to health benefits in kind or it is not possible to determine the person rights at the moment, the Competent Institution informs the Institution of Place of Stay on non-entitlement in point 4.1. of the S045. Applicable reason for the info on non-entitlement chosen, i.e.:

 “Not entitled to sickness benefits from this member state during the request period” if the person is not entitled;

 “Unknown in this situation” if it is not possible to determine the person rights;

 “Other” if none of the available reason is applicable. If this reason is chosen additional information should be provided in free text section.
