Acute kidney injury: case report
In prospective observational study of 3 men, hospitalised at a Military Medical Academy in Bulgaria between 25 November 2020 and 4 January 2021, a 58-year-old man was described, who developed acute kidney injury following immunosuppressive therapy with tacrolimus.
The man, who had a history of arterial hypertension, diabetes mellitus obesity, had undergone liver transplantation due to hepatitis C virus cirrhosis. His immunosuppressive therapy included tacrolimus [dosage and route not stated]. After 5 years of the transplantation, he was hospitalised with a 6-day history of fever, diarrhoea and dry cough. Further investigations revealed the diagnosis of COVID-19 infection. He was also diagnosed with acute kidney injury associated with tacrolimus [duration of treatment to reaction onset and outcome not stated]. He received off-label unspecified antibacterials [antibiotics], methylprednisolone, dexamethasone and convalescent-anti-SARS-CoV-2-plasma [convalescent Covid-19 plasma], in addition to remdesivir and high- flow oxygen therapy. He also received fluconazole. Subsequently, he was intubated and kept on mechanical ventilation. After 9 days of intubation, he died of respiratory failure.
