SED S044 - Request for Entitlement Document – Temporary Stay

This SED allows the Institution of the Place of Stay to request for the entitlement document for health benefits in kind during the temporary stay of a person concerned.

While indicating the time for which the SED should be valid, the Institution of the Place of Stay should give the beginning and ending date on which such certification is required. The start date should be the first day on which necessary treatment was provided in the Member State of Stay. The end date of requested entitlement document should be the expected date of end of necessary treatment provided in the Member State of Stay.

The Institution of the Place of Stay should indicate all possessed personal data of a person concerned for easier identification of a person by the Competent Institution, especially person’s PIN in the competent country, family name, forename, date of birth and address.
