Sample size calculation:
The main objective of this phase III clinical study is to assess via in-place validation whether the performance of novel functional lung MRI is sufficient to replace VQ-SPECT (the current clinical standard) to detect CTEPH, with a resulting lower diagnostic burden (functional MRI has a lower diagnostic burden than VQ-SPECT).
The study will be considered successful in demonstrating that MRI can replace VQ-SPECT if the following two hypotheses can be confirmed for the interrelation of MRI and VQ-SPECT:
(i) The probability that MRI is positive in patients who are VQ-SPECT positive and who have a positive gold standard is larger than 95% and,
(ii) The probability that MRI is positive in patients who are VQ-SPECT negative and who have a negative gold standard is smaller than 10%.
An added advantage of the MRI would be demonstrated if the following probabilities are high:
(iii) the probability that MRI is negative in patients who are VQ-SPECT positive but have a negative gold standard,
(iv) the probability that MRI is positive in patients who are VQ-SPECT negative but have a positive gold standard.
If hypotheses (i) and (ii) can be confirmed, we can conclude that MRI identifies almost all cases in the former VQ-SPECT strategy and does not increase the rate of false positive findings and, correspondingly, the number of unnecessary CPA/CTPA, which would, in turn, lead to an unacceptable increase in diagnostic burden.
An alternative strategy would be to send all study patients for a CPA/CTPA to precisely determine the sensitivity and specificity of the functional MRI, but, as outlined above, this was considered an unacceptable increase in the diagnostic burden and radiation for patients who are not usually subjected to further diagnostic interventions. A 6-month clinical follow-up visit was included in the trial in order to identify cases diagnosed as false negative in the diagnostic work-up but subsequently clinically identified in the course of the disease.
Our sample size calculation was performed in nQuery Advisor 7.0 using a Χ²-test with normal approximation. For both hypotheses, a one-sided type I error of 2.5% and a power of 80% is assumed. The sample-size calculation is based on a retrospective observational substudy from the Sheffield ASPIRE registry (Rajaram, Thorax (68), p.677f). The substudy included 132 patients with suspected CTEPH, and the prevalence of CTEPH was reported to be around 60%. For SPECT (Q-Scan), sensitivities and specificities were reported as 96% and 90%,
respectively, and as 97% and 92% for the functional MRI.
Based on this information, a simulation study was conducted with several correlation scenarios between VQ-SPECT and MRI. Due to the high sensitivities and specificities of MRI and VQ-SPECT, concordance between the two screening tests is always strong. We decided for a medium correlation of 0.5 in data simulation leading to concordances of 94% in diseased patients and 88% in non-diseased patients. A number of 5000 simulation runs with 1000 patients each resulted in the following mean frequencies (with column percentages in brackets if not specified otherwise):
For the first hypothesis, it is assumed that the true rate of patients who are positive in MRI of those who are positive in the VQ-SPECT and positively verified by gold standard is 98%. To demonstrate that this respective proportion is larger than 95%, 331 patients with positively verified VQ-SPECT are necessary. In order to find 331 patients with positive VQ-SPECT and positive gold standard, at least 571 patients need to be recruited in this study.
For the second hypothesis, it is assumed that the true rate of patients who are positive in MRI of those who are negative in VQ-SPECT and negative in gold standard is 6%. In order to demonstrate that MRI does not increase the rate of false positive findings beyond what would be accepted for VQ-SPECT by more than 10%, at least 388
patients with a negative VQ-SPECT and negative gold standard need to be recruited. To find 388 patients that are VQ-SPECT negative and negative in gold standard, 1080 patients need to be recruited.
In consequence, the latter sample-size requirement will determine the size of the study with a total of N=1080 patients, however, recruitment will proceed until 331 patients with positively verified VQ-SPECT and 388 patients with negative VQ-SPECT and negative gold standard have been included.
