K r a e p e l i n e r i n nert sich dessen, daß W i l h e l m W u n d t im J a h r e 1879 d a s I n s t i t u t f ür P s y c h o l o g i e a n d e r L e i p z i g e r U n i v e r s i t ä t g e g r ü n d e t hat. V o r l ä u f i g b e f a n d s i c h d a s s o g e n a n n t e P r i v a t i n s t i t u t in e i n e m z u r ü c k h a l t e n d e n Z i m m e r und W u n d t h a t t e nur e i n i g e treue G l e i c h g e s i n n t e . In ein paar J a h r e n b e f a n d es sich s c h o n in e i n e m d r e i s t ö c k i g e n H a u s . N a c h K r a e p e - lins T o d e s c h r e i b t W e y g a n d t in e i n e m Beitrag, daß E . K r a e p e l i n u nd C a t e l l die t r e u e s t e n A n h ä n g e r v o n W u n d t g e w e s e n s i n d /15/. In W u n d t s L a b o r a t o r i u m b e g a n n E. K r a e p e l i n s c h o n 1 8 8 2 m i t s e i n e n F o r s c h u n g e n a u f d e m G e b i e t der e x p e r i m e n t e l l e n P s y chologie. E. K r a e p e l i n u n t e r s u c h t e mit H i l f e d e r v o n W u n d t e r w o r b e n e n u nd t eils s elb s t a u s g e a r b e i t e t e n M e t h o d e n die W i r k u n g m e h r e r e r in jener Zeit g e b r ä u c h l i c h e r A r z n e i m i t t e l auf die p s y c h i s c h e n L e i s t u n g e n des M e n s c he n. S e i n e F o r s c h u n g e n d i e s e r A r t s e t z t e K r a e p e l i n a u c h w ä h r e n d des W i r k e n s in T a r t u fort (1886 - 1891), d a d u r c h a u c h s e i n e N a c h f o l g e r u n d M i t a r b e i t e r a n s p o r n e n d . A n der H e i d e l b e r g e r U n i v e r s i t ä t f a ß t e er s e i n e F o r s c h u n g s e r g e b n i s s e z u s a m m e n und v e r ö f f e n t l i c h t e sie 1892 in der M o n o g r a p h i e " Ue b e r die B e e i n f l u s sung e i n f a c h e r p s y c h i s c h e r V o r g ä n g e d u r c h e i n i g e A r z n e i m i t t e l " /8/. Die T ä t i g k e i t v o n E. K r a e p e l i n im B e r e i c h der e x p e r i m e n t e l l e n P s y c h o l o g i e u n d P s y c h o p h a r m a k o - l o g i e k a n n a u f G r u n d s e i n e r Вe t ä t i g u n g s o r te , F o r s c h u n g s r i c h t u n g e n , M e t h o d o l o g i e un d V e r a l l g e m e i n e r u n g e n in d re i P e r i o d e n e i n g e t e i l t werden.
D i e e r s t e P e r i o d e u m f a ß t s e i n e T ä t i g k e i t ( 1 8 8 2 - 1 8 8 5 ) in W u n d t s L a b o r a t o r i u m in L e i p z i g /6, 7/. A uf di e e i n i g e r m a ß e n p r o v i s o r i s c h e B e d e u t u n g K r a e p e l i n s F o r s c h u n g e n w ä h r e n d der L e i p z i g e r P e r i o d e h a t a u c h I. K a n n a b i h h in s e i n e r " G e s c h i c h t e d e r P s y c h i a t r i e " h i n g e w i e s e n /16/.
In d e r z w e i t e n s o g e n a n n t e n T a r t u e r P e r i o d e (1886 - 1891) e r r e g e n A u f m e r k s a m k e i t K r a e p e l i n s F o r s c h u n g e n in e r s t e r L i n i e d u r c h m e t h o d i s c h e V o l l k o m m e n h e i t u n d D u r c h d a c h t h e i t . S c h o n E n d e 1 8 8 6 w a r K r a e p e l i n e i n e r der O p p o n e n t e n bei der V e r t e i d i g u n g der D o k t o r a r b e i t v o n A u g u s t S h o r t , A s s i s t e n t d e r K l i n i k f ü r P s y c h i a t r i e . In d e r D i s s e r t a t i o n w u r d e die W i r k u n g v o n H y o s z i n b e h a n d e l t . D e r w i s s e n s c h a f t l i c h e L e i t e r d i e s e r D o k t e r d i s s e r t a t i o n war H. E m m i n g h a u s , V o r g ä n g e r v o n E. K ra e p e l i n .

K r a e p e l i n h a t s i c h m i t D a n k d a r a n e r i n n ert , da ß es ih m m ö g l i c h war, d i e n ö t i g e v e r v o 1 1 k o m m n e t e A p p a r a t u r für e x p e r i m e n t e l l - p s y c h o l o g i s c h e F o r s c h u n g e n i m P h y s i k l a b o r a t o r i u m d e r T a r t u e r U n i v e r s i t ä t zu b a u e n . In d e m L e i t a r t i k e l " P s y c h o l o g i s c h e r V e r su ch in der P s y c h i a t r i e " / 9 / d e s e r s t e n B a n d e s d e s v o n K r a e p e l i n h e r a u s g e g e b e n e n S a m m e l w e r k e s
" P s y c h o l o g i s c h e A r b e i t e n " e r w ä h n t er w i e d e r in v o l l e r D a n k b a r k e i t A l e x a n d e r Schmi dt , d e n U n i v e r - s i t ä t s p r o f e s s o r in Tartu, der i hm ein Z i m m e r f ü r D u r c h f ü h r u n g d e r U n t e r s u c h u n g e n z u r V e r f ü g u n g g e s t e l l t hatte.
In T a r t u b e g e i s t e r t K r a e p e l i n für e x p e r i m e n t e l l - p s y c h o l o g i s c h e F o r s c h u n g e n a u c h se i n e M i t a r beiter. U n t e r s ei n e r L e i t u n g ist 1887 die D o k t o r d i s s e r t a t i o n v o n H. D e h i o " U n t e r s u c h u n g e n ü b e r E i n f l ü s s e d e s C o f f e i n s u n d T h e e s auf di e D a u e r e i n f a c h e r p s y c h i s c h e r V o r g ä n g e " / 2 / a b g e s c h l o s sen. In d i e s e r p s y c h o m e t r i s c h e n F o r s c h u n g w u r d e Ü b e r b l i c k ü b e r d e n E i n f l u ß v o n K o f f e i n u n d T e e auf die L a t e n z z e i t e n der p s y c h i s c h e n V o r g ä n g e des M e n s c h e n v e r s c h a f f e n . 1889 v e r t e i d i g t e A. O e h r n s e i n e D i s s e r t a t i o n " E x p e r i m e n t e l l e S t u d i e n zur I n d i v i d u a l p s y c h o l o g i e " /11/. In s e i n e r D i s s e r t a ti o n b e h a n d e l t er o b j e k t i v e E i n s c h ä t z u n g s m ö g l i c h k e i t e n v o n i n d i v i d u e l l e n A b w e i c h u n g e n der E x p e r i m e n t e r g e b n i s s e . M. E i n e r b e f a ß t e s i c h in s ei ne r D i s s e r t a t i o n (1889) " E x p e r i m e n t e l l e S t u d i e n ü b e r d e n Z e i t s i n n " / 3 / m i t d e r F o r s c h u n g d e s Z e i t sinns. In d e m s e l b e n J a h r h a t A. B e r t e l l s s e i n e D i s s e r t a t i o n " V e r s u c h e ü b e r d i e A b l e n k u n g der A u f m e r k s a m k e i t " /1/ v e r t e i d i g t . In de n D i s s e r t a t i o n e n v o n M. F a l k " V e r s u c h e ü b e r die R a u m s c h ä t z un g mit H i l f e v o n A r m b e w e g u n g e n " /4/ u n d v o n G.
H i g i e r " E x p e r i m e n t e l l e P r ü f u n g der p s y c h o l o g i s c h e n M e t h o d e n im B e r e i c h d e s R a u m s i n n e s d e r N e t z h a u t " /5/ w e r d e n auf G r u n d der e x p e r i m e n t e l len B e o b a c h t u n g e n v e r s c h i e d e n e A s p e k t e d e r R a u m w a h r n e h m u n g e n behande lt . N u r n o c h in d e n e r s t e n zwei J a h r e n der H e i d e l b e r g e r P e r i o d e b e s c h ä f t i g t e s i c h E. K r a e p e l i n e i n i g e r m a ß e n m i t d e n p s y c h o p h a r m a k o l o g i s c h e n U n t e r s u c h u n g e n , v e r a l l g e m e i n e r t e u n d o r d n e t e d i e f r ü h e r e n M a t e r i a l i e n . K r a e p e l i n s ein e a u s f ü h r l i c h e n S t u d i e n der W i r k u n g v o n A l k o h o l , K o f f e i n u n d T e e u n d e b e n s o d e m E i n f l u ß d i e s e r S t o f f e auf p s y c h i s c h e V o r g ä n g e des M e n s c h e n . H. D e h i o s u n d K r a e p e l i n s e i g e n e U n t e r s u c h u n g e n h a b e n o r i g i n e l l e p s y c h o m e t r i s c h e A u s k u n f t ü b e r d i e K e n n z e i c h n u n g der W i r k u n g o b e n g e n a n n t e r S t o f f e gegeben. S e i n e e r s t e n E r g e b n i s s e in d i e s e m B e r e i c h s t e l l t e er d e r K o n f e r e n z f ü r N a t u r f o r s c h e r und M e d i z i n e r in F r e i b u r g 1883 vor. Se in e S t u d i e n w a r e n v i e l u m f a n g r e i c h e r u n d m e t h o d i s c h r e i f e r a l s d i e e r s t e p s y c h o m e t r i s c h e F o r s c h u n g v o n I.W. W a r r e n / 1 4 / , der si ch e b e n s o mit d e m E i n f l u ß A l k o h o l s befaßte.
I.W. W a r r e n b e n u t z t e nur d a s M e s s e n d e r L a t e n z z e i t d e r m o t o r i s c h e n R e a k t i o n . S e i n e V e r s u c h e w a r e n t e c h n i s c h u ne xa k t , d e s h a l b k a m er a u c h zu w i d e r s p r ü c h l i c h e n E r g e b n i s s e n . In s e i n e n F o r s c h u n g e n v e r w e n d e t e E. K r a e p e l i n M e t h o d e n d e r m o t o r i s c h e n R e a k t i o n un d der A s s o z i a t i o n s e x p e r i mente. Es ist d e u t l i c h eine t e c h n i s c h u n d m e t h o d o l o g i s c h g e n a u e r e D u r c h f ü h r u n g der V e r s u c h e und e b e n s o e i n e s o r g f ä l t i g e r e E r g e b n i s a n a l y s e zu b e m e r k e n . D o c h w a r er s e l b s t s p ä t e r mit d i e s e r V e r s u c h s d u r c h f ü h r u n g n i c h t z u f r i e d e n , d e s h a l b b e g a n n er in T a r t u e rn e u t die W i r k u n g v o n A l k o h o l zu forschen. Er w a n d t e d a b e i v e r s c h i e d e n e K o m p l e x m e t h o d e n , v e r v o 1 1 k o m m n e t e A p p a r a t u r zur M e s s ung der R e a k t i o n s z e i t e n un d p r ä z i s i e r t e A n a l y s e m e t h o d e z u r A u s w e r t u n g der E r g e b n i s s e an. K r a e p e l i n k a m zur S c h l u ß f o l g e r u n g , d a ß A l k o h o l v e r s c h i e d e n e T e i l e des p s y c h i s c h e n Z u s t a n d e s u n t e r s c h i e d l i c h b e e in fl uß t: s e n s o r i s c h e und i n t e l l e k t u e l l e F u n k t i o n e n w e r d e n g e h e m m t , m o t o r i s c h e F u n k t i o n e n w e r d e n aber a n g e r e i z t . Es w a r d a m a l s n i c h t n u r ein neuer, s o n d e r n a u c h ein die b i s h e r i g e n A u f f a s s u n g e n g r u n d s ä t z l i c h ü b e r p r ü f e n d e r S t a n dp un kt .

E. K r a e p e l i n s u n d H. D e h i o s F o r s c h u n g e n h a b e n w e g e n d e r V e r g l e i c h u n g d e r W i r k u n g v o n K o f f e i n u nd T e e ei n e n b e s o n d e r e n Wert. E. B e i d e r e x p e r i m e n t e l l e n U n t e r s u c h u n g d e r p s y c h i s c h e n V o r g ä n g e ist eine der g r ö ß t e n S c h w i e r i g k e i t e n ihre s p o n t a n e E m p f i n d l i c h k e i t h i n s i c h t li ch der ä u ß e r e n F aktoren. E. K r a e p e l i n w a r ü b e r a l l e s o l c h e d i e V e r s u c h s r e i n h e i t s t ö r e n d e n U m s t ä n d e im klaren. Er w a r i m m e r b e m ü h t , j e g l i c h e s t ö r e n d e n F a k t o r e n a u s z u s c h a l t e n . U n t e r a n d e r e m sei es h ie r erwäh nt , da ß er se i n e F o r s c h u n g e n in T a r t u h a u p t s ä c h l i c h in e i n e m für V e r s u c h e v o r g e s e h e n e n R a u m des I n s t i t u t s für P h y s i o l o g i e d u r c h g e f ü h r t hatte. Er war bes tr eb t, V e r s u c h e m e h r m a l s zu w i e d e r h o l e n , u m ein m e h r o de r w e n i g e r s t a b i l e s N i v e a u d e r E r g e b n i s s e d e r b e n u t z t e n M e t h o d e zu er zi el en , erst da n n b e g a n n er der V e r s u c h s p e r s o n P h a r m a k a zu geben. E. D i e s e r G e d a n k e n g a n g z e i g t , w a s für ei n a n s p r u c h s v o l l e r M e n s c h E. K r a e p e l i n war. A b e r se in e Ü b e r l e g u n g e n s i n d i n t e r e s s a n t a u c h v o n e i n e m a n d e r e n G e s i c h t s p u n k t aus, n ä m l i c h ist d a r i n ein e V o r s t u f e z u d i e s e m m e t h o d i s c h e n V e r f a h r e n z u be m e r k e n , w a s h e u t e in d er e x p e r i m e n t e l l e n P s y - c h o p h a r m a k o l o g i e a l s s o g e n a n n t e P l a z e b o - M e t h o d e g e n a n n t wijrd u n d w a s a l s e i n e v e r e i n h e i t l i c h e M e t h o d e f ü r die G e w ä h r l e i s t u n g de r z u v e r l ä s s i g e n V e r s u c h s e r g e b n i s s e v e r w e n d e t wird. O b w o h l K r a e p e l i n n o c h n i c h t in s e i n e n V e r s u c h e n zur A n w e n d u n g v o n P l a z e b o g e k o m m e n war, l a g e n s e i n e n A u s w e r t u n g e n ü b e r d i e W i r k u n g v o n P h a r m a k a i m m e r z a h l r e i c h e K o n t r o l l v e r s u c h e o h n e A r z n e i m i t t e l z u g r u n de. D i e D i s s e r t a t i o n v o n A. Oehr n, d ie er u n t e r L e i t u n g v o n K r a e p e l i n a b g e s c h l o s s e n h a t , e n t h ä l t w e r t v o l l e A n g a b e n zur i n d i v i d u e l l e n V a r i a b i l i t ä t der N o r m w e r t e der U n t e r s u c h u n g s m e t h o d e n . E. K r a e p e l i n v e r w e n d e t e d i e s e A n g a b e n als K o n t r o l l g r u p p e bei a n d e r e n F o r s c h u n g s a r b e i t e n .

A m 20 O k t o b e r 1888 / 12 / fa ß t e er in e i n e m B e r i c h t au f d er w i s s e n s c h a f t l i c h e n S i t z u n g d e s R a t e s d e r M e d i z i n f a k u l t ä t de r T a r t u e r U n i v e r s i t ä t s e i n e V e r s u c h s e r g e b n i s s e ü b e r b l i c k - li ch zusam me n. Es s t e l l t e s i c h he ra us, d a ß in d e n e r s t e n T a g e n d i e V e r s u c h s e r g e b n i s s e si c h z i e m l i c h s c h n e l l v e r b e s s e r t e n , d a n n s c h o n et w a s l a n g s a m e r u n d u n g e f ä h r ab des s e c h s t e n Tage s, an d e m k l e i n e r e z u f ä l l i g e A b w e i c h u n g e n u n d V e r ä n d e r u n g e n im T a g e s a b l a u f , e b e n s o M ü d i g k e i t b e m e r k b a r w u r d e n , e r r e i c h t e n sie e in g e w i s s e s s t a b i l e s Niveau. K r a e p e l i n d e m U n t e r s u c h t e n i r g e n d e i n e A r z n e i zu geben, u n d das f o l g t e in A b w e c h s l u n g m i t d e n s o g e n a n n t e n " l e e r e n T ag en ", a n d e n e n m a n n i c h t d ie A r z n e i m i t t e l gab. O f t d a u e r t e n die V e r s u c h s s e r i e n lange, bis 17 Tage, d i e G e s a m t z a h l d e r V e r s u c h e b e t r u g 150. D a s a l l e s b e k r ä f t i g t e d i e T a t s a c h e , d a ß K r a e p e l i n s V e r s u c h s e r g e b n i s s e im W i d e r s p r u c h zu d e n b i s h e r i g e n F o r s c h u n g s e r g e b n i s s e n standen, a b e r u n g e a c h t e t d e s s e n h i e l t e n sie d er k ü n f t i g e n Z e i t p r o b e stand.
E r w a r e i n e r d e r e r s t e n , d e r e s f ü r s e h r w i c h t i g h i e l t , da ß bei d e n e x p e r i m e n t e l l - p s y c h o
l o g i s c h e n U n t e r s u c h u n g e n e i n e b e s o n d e r e A u f m e r k s a m k e i t d e m B e f i n d e n u n d d e m i n n e r e n Z u s t a n d des P a t i e n t e n g e l t e n soll.
E. K r a e p e l i n b l i c k t e in die Z u k u n f t der e x p e r i m e n t e l l e n P s y c h o l o g i e o p t i m i s t i s c h . Er h o f f t e sehr v i e l v o n der F o r s c h u n g ü b e r W i r k u n g de r A r z n e i m i t t e l , die e i n e n f r u c h t b a r e n E i n f l u ß s o w o h l a u f d i e P s y c h o l o g i e al s a u c h auf d i e P h a r m a k o l o gi e h a b e n sollte.

It is not m e r e l y a n i n t e l l i g e n c e test; it is a m e t h o d of s t a n d a r d i z e d i n t e r v i e w In the f i e l d of d e p r e s s i o n and a n x i e t y this s t a t e m e n t is v a l i d for the H a m i l t o n scales. T h e two m o s t u s e f u l m e t h o d s in th is r e s p e c t h a v e b e e n f a c t o r a n a l y s i s [16; 37] an d la te nt s t r u c t u r e a n a l y s i s [21; 34]. B o t h p s y c h o m e t r i c m e t h o d s h a v e b e e n a p p l i e d to the H a m i l t o n scales.
2. F a c t o r a n a l y s i s of the H a m i l t o n D e p r e s s i o n S c a l e (HDS)
W h e n H a m i l t o n [23] p u b l i s h e d hi s d e p r e s s i o n s c a l e he u s e d fa c t o r a n a l y s i s to d e f i n e the d i m e n s i o n of s e v e r i t y of d e p r e s s i v e states. H o w ever, no g e n e r a l fa c t o r wa s found. In hi s s e c o n d s t u d y w i t h the H DS a g e n e r a l fa c t o r of s e v e r i t y w a s fo un d [24] . As p o i n t e d out by H a m i l t o n h i m self [27] the r e a s o n for t hi s d i s c r e p a n c y b e t w e e n the two s t u d i e s w a s that the h e t e r o g e n e i t y of the p a t i e n t s r e g a r d i n g s e v e r i t y of d e p r e s s i o n was l ar ge r in the s e c o n d t h a n in the f irst study.
T h i s is one of the d r a w b a c k s of the fa c t o r a n a l y t i c m e t h o d that it is s e n s i t i v e to the

d i s t r i b u t i o n of the p o p u l a t i o n u n d e r i n v e s t i g a tion. W h e n s u m m a r i z i n g the m a n y s t u d i e s that have b e e n c a r r i e d out on the HDS w i t h f ac t o r a n a ly si s, H a m i l t o n [27] has s tat ed that mo st i n v e s t i g a t o r s w o u l d a c c e p t two f act o r s in the scale, a g e n e r a l fac t o r of s e v e r i t y and a sec o n d for a n x i o u s - a g i t ated d e p res si on .

As the b r i g h t e r p e r s o n is a ble to r e s p o n d a d e q u a t e l y to mo st of the items w h i l e the d u l l e r p e r s o n o n l y is a bl e to r e s p o n d a d e q u a t e l y to the less d i f f i c u l t items, the total sco re of a d e q u a t e i tems r e s p o n s e is a s u f f i c i e n t s t a t i s t i c of the d i m e n s i o n of i nt el li g e n c e . H a m i l t o n [26] m ad e an a t t e m p t to a n a l y s e his d e p r e s s i o n sc ale by the G u t t m a n s c a l o g r a m p r i n c i p l e but f o c u s s e d o n l y on a fe w items.
We h av e us e d the latent s t r u c t u r e a n a l y s i s d e v e l o p e d by R a s c h [34] w h e n we v a l i d a t e d the HDS [7]. T h i s R a s c h m e t h o d is a f u r th er c o n s t r u c t of the G u t t m a n s c a l o g r a m a n a l y s i s t a k i n g p r o b a b i l i s tic t h e o r i e s of er r o r s of m e a s u r e m e n t into a c count. We found that the s t r u c t u r e of s e v e r i t y of d e p r e s s i v e st a t e s in the HDS c ou ld be s u f f i c i e n t ly d e f i n e d by six i tems (i nd i c a t e d by a the le t t e r s a to f in T a b l e 2). On the b asis of these si x H DS i tems we [5] d e v e l o p e d the M e l a n c h o l i a S c a l e (MES) w h i c h has a h i e r a r c h i c a l s t r u c t u r e in the se nse of G u t t m a n as s h o w n in T a b l e 1. P a t i e n t s w i t h m i n o r d e p r e s s i o n or d y s t h y m i a scored on the first items, w h i l e p a t i e n t s w i t h m a j o r d e p r e s s i o n a l s o s co red on items of guilt, mo t o r a c t i v i t y and s u i c i d a l t h o u g h t s (Table 1).

4. C o n s e q u e n c e s of the p s y c h o m e t r i c s t u d i e s w i t h HDS
In T a b l e 1 is sh o w n the p s y c h o m e t r i c d e v e l o p me nt in the c l a s s i f i c a t i o n of d e p r e s s i o n f r o m the ICD s y s t e m (e.g. ICD- 10 ), the D S M - I I I - R s y s t e m and the M ES m o d i f i c a t i o n of HDS.
T he ICD s y s t e m is ba s e d on the c l a s s i c a l t e x t b o o k c l a s s i f i c a t i o n w h e r e no s t r u c t u r e is needed. T he D S M - I I I - R is ba se d on the p r o t o t y p e a l g o r i t h m m e a n i n g that the s y m p t o m s i n d i c a t e d in T a b l e 1 h av e a h i g h e r m u t u a l c o r r e l a t i o n that t h e y do w i t h ot h e r s y m p t o m s in the D S M - III -R . T h i s p r o t o t y p i c a p p r o a c h [13] is i d e n t i c a l w i t h the p r i n c i p l e of f a c t o r s in fa ct o r a n a ly si s. The la te nt s t r u c t u r e a n a l y s i s in an a p p r o a c h i d e n t ical to the c o n c e p t of t r a n s p a r e n c y u s e d by F e i n s t e i n [15] w h i c h r e f e r s to the a b i l i t y to see t h r o u g h ea c h level of to ta l s co r e s of a r a t i n g sc a l e to d e t e r m i n e w h a t it co ntains. As i n d i c a t e d in T a b l e 1 a total s co r e of M E S of 0 to 5 m e a n s no d e p r e s s i o n ; a sc o r e of 6 to 14 m e a n s m i n o r d e p r e s s i o n or d y s t h y m i a ; and a s co re of 15 or m o r e m e a n s m a j o r d e p r e s s i o n . T he d i m e n s i o n of s e v e r i t y of d e p r e s s i o n in the H DS as d e f i n e d by R a s c h a n a l y s i s has b e e n s u p p o r t e d by e x t e r n a l v a l i d i t y studies. It c o r r e l a t e d w i t h the g l o b a l a s s e s s m e n t p e r f o r m e d by e x p e r i e n c e d p s y c h i a t r i s t s [6] . P r e t r e a t m e n t l y , the c l a s s i f i c a t i o n of p a t i e n t s w i t h p an ic d i s o r d e r i nt o no, m i n o r and m a j o r d e p r e s s i o n had p r e d i c t i v e v a l i d i t y c o n c e r n ing r e s p o n s e to i m i p r a m i n e v e r s u s p l a c e b o [10], as no d i f f e r e n c e w a s s e e n in p a t i e n t s w i t h o u t d e p r e s s i o n w h i l e i m i p r a m i n e w a s s u p e r i o r in d e p r e s s e d pa tients.
W h e n u s e d as o u t c o m e m e a s u r e of a n t i d e p r e s s i v e t r e a t m e n t a n e g a t i v e c o r r e l a t i o n b e t w e e n total s c o r e s of the m e l a n c h o l i c s c o r e s and p l a s m a l e v e l s of c l o m i p r a m i n e w a s found p o s t t r e a t m e n t l y

A sc a l e d e v e l o p e d to be s e n s i t i v e to c ha n g e d u r i n g p s y c h o p h a r m a c o l o g i c a l t r e a t m e n t by M o n t g o m e r y and Ä s b e r g [32] (MADRS) is i n c l u d e d in the H D S / M E S u n i v e r s e as we ll (Table 2). T hi s sc al e seems less s e n s i t i v e to c h a n g e d u r i n g t r e a t m e n t c o m p a r e d to M E S [20]. U s i n g 50 % r e d u c t i o n of p r e t r e a t m e n t s co re the M A D R S w a s f ound less s e n s i t i v e t h a n HDS [4]. F u r t h e r m o r e and M a i e r h a v e c o r r e l a t e d the H D S / M E S v e r s i o n of M A R D S (Table 2) w i t h the o r i g i n a l M A D R S v e r s i o n in a s tu d y i n c l u d i n g 118 d e p r e s s e d patients. A c o r r e l a t i o n c o e f f i c i e n t of 0.91 w as obtai ned . The i n t e r c e p t of the r e g r e s s i o n line wa s 1.6 w i t h a sl o p e of 0.9. In ot h e r words, a s core of 10 on the M A D R S c o r r e s p o n d s to 10.6 on the H D S / M E S v e r s i o n ; a s core of 20 on M A D R S c o r r e s p o n d s to
19.6 on the H D S / M E S ver si o n ; etc.
L i k e wi se , we ha ve found that a s cor e of 15 or m o r e on the M ES c o r r e s p o n d s to a m a j o r d e p r e s s i o n on D S M - I I I [8]. Th e g r o u p of items d e f i n i n g the D S M - I I I c o n c e p t of d y s t h y m i a is al s o i n c l u d e d the u n i v e r s e of it ems in T a b l e 2.
T he u n i v e r s e of items in T a b l e 2 has, h owever, not a c o v e r a g e of items s p e c i f i c a l l y for the d e p r e s s i v e t r i a d e of n e g a t i v e s e l f - e v a l u a t i o n of past, present, and future. T he first scale that c o v e r e d the se items wa s the W e c h s l e r et al. scale [39]. T h i s scale has, h ow ev er, not b ee n f r e q u e n t ly u s e d in c l i n i c a l research.
5. F a c t o r a n a l y s i s of the H a m i l t o n A n x i e t y S c a l e (HAS)
In his first st u d y w i t h this scale H a m i l t o n [22] found a g e n er al fa ct or as the first fa ct or and a bi po l a r fa ct or as the se co nd (psychic v e r s u s so ma t i c a n x i e t y ) . In his se c o n d s tud y th es e f i n d i n g s w e r e c o n f i r m e d [25]. In b oth s t u d i e s H a m i l t o n i n v e s t i g a t e d p a t i e n t s w i t h a n x i e t y ne uroses. W e hav e recen tl y, in a p o p u l a tion of p a t i e n t s w i t h panic d i s o r d e r s [11] r e p l i cated the f i n d i n g s of Hamilton.

6. L a t e n t s t r u c t u r e a n a l y s i s of the H a m i l t o n A n x i e t y Sc a l e
W h e n u s i n g the H A S o u t s i d e its o r i g i n a l s cope ( a n x i e t y n e u r o s i s ) we f oun d that the f a c t o r of p s y c h i c a n x i e t y w a s m o s t r e l e v a n t in d e p r e s s i v e i l l n e s s [18]. In p a t i e n t s e v a l u a t e d b e f o r e c a r di a c b y p a s s s u r g e r y we fo u n d [9] a h i g h c o r r e l a ti o n b e t w e e n HA S and M E S (R = 0.91). In the l a t t e r s t u d y o n l y 3 % had a m a j o r d e p r e s s i o n w h i l e 63 % had a m i n o r de pr e s s i o n .
In a s t u d y u s i n g R a s c h a n a l y s i s M a i e r et al.
[30] fou nd that the p s y c h i c a n x i e t y f a c t o r of HAS w a s m o s t l i k e l y to f u l f i l l the R a s c h c r i t e r i a th a n the t otal HAS or the s o m a t i c a n x i e t y factor.
T h i s f i n d i n g is in h a r m o n y w i t h a R a s c h a n a l y s i s c a r r i e d out in p a t i e n t s w i t h p an ic d i s o r d e r s
[1 1] .
7. T h e u n i v e r s e of HDS, MES, HAS
In T a b l e 2 is sh o w n the r e l e v a n t i tems of the HA S in r e l a t i o n to the H D S / M E S u ni v e r s e . Of these it em s d e p r e s s e d mood, p s y c h i c an xie ty , i n s o m n i a g e n e r a l and i n t e l l e c t u a l r e t a r d a t i o n are i d e n t ical to the c o r r e s p o n d i n g MES items that c ov e r s m i n o r de pr e s s i o n . T he o r i g i n a l HAS i t e m of g e n eral m u s c u l a r s y m p t o m s c o r r e s p o n d s to the ME S i t e m of t i r e d n e s s and pains. T he o r i g i n a l HAS it e m of b e h a v i o u r at i n t e r v i e w c o r r e s p o n d s to the HDS i t e m of ag it a t i o n , but it se ems that this it e m s h o u l d not be i n c l u d e d in the d i m e n s i o n of s e v e r i t y [11]. T h e H DS i t e m of s o m a t i c a n x i e t y can be c o n s i d e r e d as a f u n e r a l i t e m of a u t o n o m i c h y p e r a c t i v i t y w h i c h m i g n t be of i m p o r t a n c e in d e s c r i b i n g the m i n o r d e g r e e s of d e p r e ss ion . T wo i m p o r t a n t HAS items, t e n s i o n and phobia, sh oul d be i n c l u d e d to co v e r the w h o l e d i m e n s i o n of anxiety. In c o n c l u s i o n , t h e r e f o r e , we have i n c l u d e d t e n s i o n and p h o b i a in the H D S / M E S u n i v e r s e to cov e r a l s o the HAS. T he e ig ht HAS items should be f u r t h e r a n a l y s e d by R a s c h m o d e l s to test w h e t h e r t he y d e s c r i b e mi l d d e g r e e s of mi n o r d e pressi on .
8. T est of time of the H a m i l t o n s ca l e s
It has o f t e n be e n st at ed that it was the i n t r o d u c t i o n of a n t i d e p r e s s a n t s the y e ar s b e f o r e the p u b l i c a t i o n of the H a m i l t o n s ca l e s w h i c h

i n d u c e d the n e e d for s c a l e s m e a s u r i n g o u t c o m e of t r e a t m e n t like t h o s e c o n s t r u c t e d by Hamilton.
H o w e v e r , it is o f t e n o v e r l o o k e d that the H a m i l t o n s c a l e s t h e m s e l v e s have, if p r o p e r l y a d m i n i s t e r e d , a “t h e r a p e u t i c “ effect. B e f o r e the i n t r o d u c t i o n of a n t i d e p r e s s a n t s S k o t t o w e [36] in his i n t e g r a t e d t e x t b o o k of p s y c h i a t r y m a d e the f o l l o w i n g o b s e r v a t i o n : .. T h e i n i t ia l p s y c h i a t r i c i n t e r v i e w is a l w a y s imp o r t a n t , but in no g r o u p of
i l l n e s s e s it is of g r e a t e r i m p o r t a n c e as a first st e p in t r e a t m e n t th a n it is in the de p r e s s i o n . It is in the h i s t o r i c a l v a l i d a t i o n i n t e r e s t i n g to n o t e th at in the c o n t r o l l e d c l i n i c a l t r i a l s on a n t i d e p r e s s a n t s y m p t o m r a t i n g s c a l e s h a v e o u t d a t e d p s y c h o l o g i c a l t e s t s [33]. In an o v e r v i e w on a n t i d e p r e s s a n t s d r u g t r i a l s p u b l i s h e d b e t w e e n 1964 a n d 1986 in e l d e r l y d e p r e s s e d pa ti en ts, G e r s o n et al. [17] h a v e s h o w n that the H a m i l t o n D e p r e s s i o n S c a l e o u t d a t e d the c o n t e m p o r a r y s ca l e s a r o u n d 1976.
W h e n m a k i n g a e e t a - a n a l y t i c r e v i e w of a n t i d e p r e s s a n t d r u g s (Bech, 1988) it b e c a m e c le a r that t h e r e ex i s t m a n y d i f f e r e n t v e r s i o n s of the H a m i l t o n D e p r e s s i o n Scale. T h e o r i g i n a l i t e m d e f i n i t o n s w e r e o f t e n i n a d e q u a t e an d i n c l u d e d v e r s i o n s s e p a r a t i n g m a l e s and f e m a l e s whic h, of co ur s e , h a v e h i s t o r i c a l reasons. It s e e m s that the m o s t f r e q u e t n l y u s e d v e r s i o n s of the H a m i l t o n s c a l e s ar e t h o s e p u b l i s h e d by Guy, o f t e n r e f e r r e d to as the N I M H v e r s i o n s . Ho w e v e r , H a m i l t o n n e v e r a c c e p t e d t h e s e v e r s i o n s w h i c h he c o n s i d e r e d as c h e c k - l i s t s c a l e s [27]. T h e o p e r a t i o n a l c r i t e r i a that w e r e c e n t l y h a v e p u b l i s h e d w e r e d e v e l o p e d in c o l l a b o r a t i o n w i t h H a m i l t o n [10].
9. C o n c l u s i o n
Th e u n i v e r s e of it e m s c o v e r e d by the H a m i l t o n D e p r e s s i o n and A n x i e t y S c a l e s is an i m p o r t a n t g u i d e for the d o c t o r m a k i n g the i n i t i a l p s y c h i a -

trie i n t e r v i e w w i t h the a n x i o u s or d e p r e s s e d patie nts . Li ke the Bi n e t s c a l e s for m e a s u r i n g i n t e l l i g e n c e the H a m i l t o n s c a l e s can be c o n s i d e r e d as a m e t h o d of s t a n d a r d i z i n g a c l i n i cal interview.

V a r i o u s v a l i d i t y s t u d i e s ha v e c o n f i r m e d this g e n e r a l d i m e n s i o n of s e v e r i t y of d e p r e s s i o n , wh ic h, inter alia, is r e l e v a n t w h e n c o m p a r i n g the r e l a t i v e p o t e n c y of a n t i d e p r e s s a n t drugs. A m o n g the s e c o n d o r d e r d i m e n s i o n s the a n x i e t y f ac tor an d the sl e e p f a c t o r h a v e b e e n m o s t i m p o r t a n t w h e n d e s c r i b i n g the p r o f i l e of a n t i d e p r e s s a n t s .
In the H a m i l t o n A n x i e t y S c a l e t he r e ar e two m a i n f a c t o r s of w h i c h the p s y c h i c a n x i e t y fac t o r i n c l u d e s items r e l e v a n t for m i l d d e p r e s s i o n or d y s t h y m i a .
B o t h the a n x i e t y sc a l e and the d e p r e s s i o n s c a l e w e r e r e l e a s e d by H a m i l t o n w i t h r a t h e r i n s u f f i c i e n t o p e r a t i o n a l d e f i n i t o n s w h i c h h a v e r e s u l t e d in m a n y a t t e m p t s to m a k e r e v i s e d i t e m d e f i n i t o n s .
