SED S077 – Cancellation of Entitlement Document - Former Frontier Worker - Family Member of Former Frontier Worker
The institution of the Member State of residence uses this SED to inform the competent institution about the cancellation of Entitlement Document for a former frontier worker or for a family member of former frontier worker.
The following data are important here:
Status of the Person is mandatory and is completed by selecting the appropriate check-box for the status of the person concerned. Only one radio-button can be selected;
o 01 - Former Frontier Worker
o 02 - Family member of a former frontier worker
Entitlement document which is to be cancelled: the choice S008 / S3 must be completed with the information about the entitlement document previously received for the person
concerned;
"Issued on" must contain the date on which the competent institution issued the SED S008 or the portable document S3 for the person concerned;
Reason for Cancellation is mandatory and contains information about the reason for cancellation of the entitlement for the person concerned; Only one reason can be selected.
The possible choices are :
o No longer entitled to benefits in kind from our Member State
o Insured on the basis of an activity as an employed or self employed person o Other reason
Date of cancellation is mandatory and contains the date as of which the person concerned is no longer entitled to benefits in kind in the former Member State of employment at the expense of competent institution;
It is possible to add attachments to SED S077.
